I replied earlier to the list, but the attached pdf (and the email must have
been kicked out by the mail server...
In acrobat pro, you can add a digital signature field, that when the user
selects the field, they will be prompted through a signing process.
The form is simply a pdf, with an embedded control. In pdf pro, the
designer simply adds extra controls over the pdf image, for edit boxes,
radio buttons, or whatever they want. I assume the control properties are
stored at the end of the file, so that other reader would not break when the
data is encountered. The reader then displays the pdf, and then draws the
controls over the top.

I have a question for people who've made or used these forms. How does
one incorporate their signature? This especially is geared toward
government ones since I've never seen one that does't require that you
'John Hankock' the document.
