I did find the following in my notebook.  It might be useful.

Yast > Network Devices > Modem

Yast finds a modem:
	USRobotics 56K FAX INT v5.22.65
	Not configured
	Click on "Configure" /"edit"
	Modem Parameters
	Modem Device: /dev/ttys14
	Click "new" on Select ISP Page
	Provide Parameter Page: fill in Provider, Phone Number, Authorization
	DNS: if ISP has provided
	Configure as modem0
